WASHINGTON D.C. — As the House Ways & Means Committee convenes to address critical issues surrounding home care policy and access, the Alliance for Long-Term Care Pharmacy @ Home urges Congress to take decisive action by calling on CMS to leverage their authority to ensure Medicare beneficiary access to LTC pharmacy at home services. The Alliance, representing a coalition of long-term care pharmacies, professional associations, and care management groups, emphasizes the vital role of these services in supporting medically complex patients, particularly those in underserved and rural communities
Home health patients enrolled in a medication adherence program experienced a hospitalization rate that was 73.1% lower than those outside of the program, according to a new study. The medication therapy and risk management program is designed to keep patients healthier at home with fewer emergency room and hospital visits and fewer adverse health events
As high-acuity Medicare beneficiaries continue to shift towards at-home care, the need to access LTC pharmacy at-home services is growing. These services reduce adverse drug events, decrease emergency department visits, curb costs, and enable beneficiaries to access care outside of a facility setting. Today, the Alliance for Long-Term Care Pharmacy at Home is acting upon its mission to care for this population in the release of two new policy documents outlining a federal approach to supporting whole-person care to those who need it most.
